We are currently seeking experienced and dedicated Licensed Practical Nurses (LPNs) for full -time and per-diem opportunities at Casa Real in New Mexico. In this role, you will work under the direction of a Registered Nurse (RN) and play a key role in delivering clinical care and supporting patient recovery in a long-term care or rehabilitation setting.

Location: Santa Fe, NM (Local Only)
Employment Types: Full-time, Per-Diem
Pay Rate: $38/hour (Paid Weekly)
Duration: Guaranteed 36 hrs/weekly (5-14 week contracts)
Available Shifts: (Multiple Openings)
6:00PM - 6:30 AM
3 days a week (3 x 12/hrs)
Per-diem and full-time options available
Key Responsibilities:
Perform patient assessments and monitor vital signs
Administer medications and treatments per physician orders
Assist with wound care, catheterization, and other clinical procedures
Document care accurately in electronic health records (EHR), such as Point Click Care (PCC)
Supervise and support unlicensed staff to ensure quality care delivery
Collaborate with RNs, physicians, and therapists to implement care plans
Participate in discharge planning, patient/family education, and interdisciplinary meetings
Adhere to infection control protocols and maintain a safe care environment
Support patients' rehabilitation goals through compassionate, person-centered
Work Conditions:
Ability to lift, push, pull, or carry up to 40 lbs.
Frequent standing and walking during 12-hour shifts
Use of personal protective equipment (PPE), including gowns, gloves, and masks
Indoor clinical environment with potential exposure to common healthcare-related conditions (e.g., noise, odors, disinfectants).
Compliance and Ethics
Uphold all applicable healthcare regulations including HIPAA
Maintain confidentiality and protect sensitive health information
Comply with standards of conduct and professional ethics
Participate in orientation, training, and quality improvement activities as needed
Qualifications:
Graduate of an accredited Licensed Practical Nursing (LPN) program
Active LPN license in New Mexico (or applicable state)
Current BLS/CPR certification
1+ year of experience in a skilled nursing, rehabilitation, or long-term care setting preferred
Proficiency in EHR documentation (e.g., Point Click Care)
Strong communication, critical thinking, and teamwork skills
